It's working as intended for me. If installed correctly Ashley should be wearing her alliance fatigues - she's not in armor because armax defaults to her 1st outfit.
Make sure you installed the DLC mesh.

One of my favorite mods. Would it be possible for this to incorporate Getorex's hospital shirt mesh mod, though? The vanilla appearance (a shirt painted over her nude model) is laughable.
The only other change I could think to make would be to give her arms like the "Buff FemShep" mod. After installing that, just about every woman in the game looks better, but Ashley's arms are now rail-thin compared to everyone else.

Thank you!
And I wish buff femshep had come out before I made this! I would have looked into using it. But after the fact it would be some work to go back, weight it for Ash, and replace it everywhere.
Same with Getorex's mod unfortunately, the hospital body and head is one model which is completely replaced here, so it's not a simple swap. I'd have to go back, take his assets and combine them with mine to make a new model.
Anything else like hair, and it would be an easy overwrite in Me3Explorer. Right now I have no plans to do major overhauls with this mod, outside of updating the installers to make sure they work with the current version of ME3Explorer.

I'm looking into switching her default uniform body for the same thing from Buff FemShep. Model looks to have gone in fine with PSK export/import, but the texture is quite a bit darker than it should be when I look at her arms. I'll have to play around with it. Could probably do the same with the Buff FemShep "nude" model, but that will put her bra back on, which will likely cause texture issues. Did you make any changes to the mesh for Ashley's nude model, or was that simply a retexture/bump map change?
Using your face mesh for Ashley, someone put together two new armors to replace the "dress" models (I see that you replaced her default dress in combat with her unarmored fatigues).

It isn't packaged in .mod format, just UPK and DSS files. Swaps have to be done manually in ME3Explorer (I'm still figuring out how to do that). I'm hoping, ultimately, to get a final state where Ashley's noncombat appearance is in her "buff FemShep" uniform, and her combat armor in slot 00 is the Ajax variant at the link, with the Collector's Edition pink dress replaced with the second armor seen at that link. Not sure how to package this all up in a .mod, though.

I might look into helping Bomma out, whose releasing the default outfit ajax replacer you linked and see if I can cook up some .mods. Hopefully one that only replaces her outfit in combat scenario's / where it makes sense so the fatigues can be left intact on the Normandy and the likes. Keep an eye out on that release thread, i'll open a dialogue about it with them soon =)

Thanks. Between what you, ELE08, Getorex and Bomma have put together, Ashley is an entirely different character.
I was able to selectively replace the mesh for Ashley's combat 00 armor, leaving her in her fatigues in noncombat scenarios, but I accidentally overwrote the texture used for her noncombat fatigues, and the texture change wasn't applied to her appearance in the Armax Arena. *sigh* Starting over... think I need a new treescan to pick up DLC, still can't find the texture for the Collector's Edition variant to replace the pink dress.
Gonna have to move on to incorporating the Buff FemShep changes after this.
