Should I just make a fresh install?

Well, I always assume a poster wants to salvage their current game. BOSS not working properly is a concern, crashes with a clean profile is a concern. If you are willing, a re-install would be perfect.
I would:
0. Make sure you have the latest version of all your mods and MO. Put them somewhere safe. Don't tweak the default ini files unless a mod install direction tells you to. Turn features OFF or use Application Settings only in your video card control panel.
1. Delete the entire Skyrim game directory and re-install Skyrim and the DLC.
2. Install an ENBseries. You don't have to USE it. ENBseries contains memory management features. You can disable the graphics processing by editing enblocal.ini and setting: UsePatchSpeedhackWithoutGraphics=true
3. Install SSME or SKSE with Sheson's memory patch. (Not both)
4. Remove and re-install the latest MO. Run it and carefully setup your tools (BOSS, Wrye Bash, TES5EDIT) in MO.
5. Clean update.esm and the DLC.
6. Use MO to install ALL mods from here on out.
7. Add the latest Unofficial Patches, run BOSS, make SURE BOSS sorts those UOP esp's correctly. Start a new game and stress test it a bit. With ENB and SSME (or the patched SKSE), the game should not crash during the stress test.
8. Install a mod(s), run BOSS and clean the mod(s) if necessary, make a bashed patch if the mod(s) has BASH Tags in the BOSS log, install any compatibility patches. Test in-game.
9. Goto step 8 until all mods are in and working. If you don't test each mod (or a few mods at once), you won't find problems and you'll be back in the same situation you are now in; not knowing what caused a problem.
10. Start a new game once all your mods are in.
11. Carefully apply any personal ini tweaks and graphics settings.
If you get a problem, go to the mod page on the Nexus and look in the COMMENTS or FORUM to see if there's a fix.

I can actually launch and play the game now, yay! And it all looks beautiful and I can play with flawless FPS as long as I drop down some draw distances.
I have a new issue though, in that my game either freezes and sits there forever or just CTD's about 3 minutes into play. This is my new load order.
Note I am running a fair amount of high texture mods, such as Skyrim HD, Hectrols Caves, Lush Trees and Grass etc and after some research it looks like those could be making the engine cry.
