Ugh, I honestly don't know the answer... I HAVE known the answer, but I can't recall anymore. Can really npcs use the tail slot unless they are khajiits or argonians? I thought the player was the only one who could do this?
Maybe you should ask the mod author of the mod that uses tail slots instead? Or maybe Apachii, with her broad experience (she is still modding). I've never been very knowledgeable when it comes to clothing and armor.

2) Oh okay, I didn't realize I had to lose 2% of my health for her to aid me when I have her set to that mode. And oh they attack me, but they prioritize her - meaning they'll only turn their attention to me if I take out enough of their health whilst they gang up on Vilja. Otherwise, they do not see me as a threat and I can literally sit there and watch them gang up on her.
So what do you propose I do? I doubt it's Deadly Reflex, because I actually had her working properly for a month or two before this. Do you propose I just disable and re-enable the mod? Do you think that might solve it?

So you do notice a behaviour in enemies' approach to her now compared to previously? It means something has changed in your game. Either you have added a new mod that changes enemy behaviour, and you think that you have not, or Vilja could be effected by a spell/curse that has lowered her personality (and thereby made enemies more "attracted" to her... iin a bad way).
Open the console and click on Vilja, so that her formID shows up at the top of the console.
Type:
GetAV personality
The value should be at least 80, which is what she has in the construction set.
If it is lower, she is effected by a spell. I think in order to remove the spell effect from her through console you have to know the formID of the spell, so it's probably easier that you try and a) cast Vilja's attack spell on *Vilja* to reset as much as possible on her, and if this doesn't help, use one of your own restoration spells to remove the spell effect from her - I don't recall the names of the spells from the top of my head, but there are some that removes spell effects from others.
part 2) of your question seems to have been cleared already, and that's good

So I've been doing some testing all morning:
I checked her personality via console and it's set at 80.06, so that doesn't seem to be the problem. I unchecked every single esp other than the officials, and the problem persisted.
However, I have narrowed down the problem. It seems the factions added by the official add-ons are the ones acting this way. I tested the vanilla enemies and they seem to be working properly. The bandits added by the Orrery add-on and the Drothmeri faction added by the Mehrune's Razor add-on are not working properly, especially the Drothmeri (these are the guys I can literally sit back and watch them gang up on Vilja). I've been doing those two add-ons most recently, so that is why it appeared to me all the enemies were acting this way. Is this a known problem? I know you've said your attempts to let enemies focus on the player instead of Vilja don't work with added factions, so does that apply to all the factions added by official content, as well?

Well, we ensured that it would work properly with Shivering Isles and, I believe, with Knights of the Nine (I don't have the construction set available right now so I can not check), and we ensured it would work with OOO, MMM etc. BUT I don't think we did anything with enemies from the small DLCs.
With a character who has normal health and normal armor, the loss of hp should be such that Vijla would run to your assistance before she healed you. But if you have this high health and good protection, it could indeed be like you say.

Since I only wanted the tail slot for clothing, I ended up sticking the skirt meshes with the pants, and now Vilja is finally looking killer: http://i.imgur.com/La7BsfL.jpg

Wow, Emma!! Superb work!! Such care and attention to detail. The best companion mod by far. I really appreciate the hundreds (thousands?) of hours it must have taken you to create Vilja. She is such a joy! Thank you so much!!
