Ok, I've experimented more. My current settings have me as essential, ragdoll on death, health bleedout percent at 50%, no health buffer, instant kill percent at 1000%, and I uninstalled DA-Survival. I've figured out what the problem is.
If I have the bleedout percent at 0%, or anything low enough that bleedout threshold can be crossed and my health go to 0 in one attack, then I die. The tutorial boxes still appear, bleedout bar still shows, it tells me I have life left in me, but the death sound is playing and without using SkyTweak to extend the death reload time, it loads my last save after a minute.
If the bleedout percent is high enough that one attack can cross the threshold without killing me, the mod works perfectly. It seems that even if I'm essential (and I think sometimes when I die I do go into the knelt stance, but it's hard to tell with the camera angle sometimes), if my health goes to 0, I die, even if the mod works otherwise. I've checked with player.isessential and confirmed that I am set to essential, although as I said in my last post I stop being essential on going into bleedout (at least sometimes).
I'm glad I found a way to make it work to some extent, but I'd rather not have to go into bleedout at 50% health every time, especially since I'm playing without regenerating health.
So why would this be happening? Script lag? I don't see any signs of that being a problem otherwise. Sorry for making such long posts, I want to make sure you have all the information needed, since "It doesn't work, help!" is useless.
TL;DR
If my health goes to 0, even while essential, I die even though the mod continues to work normally. If I set the bleedout threshold very high, the mod works as intended, because the bleedout kicks in before my health goes to 0.

Seems to me, like I think you said, that the essential flag is removed sometimes. That's not a normal DA operation -- what I mean is that DA doesn't toggle the flag during normal processing of defeat scenario. Otherwise it appear that DA is triggering like it normally would, but that something else triggers along OnBleedout to kill the player. To me, it points to another script messing with that but IDK which mod.
That problem is exactly why I created DA in the first place, one can't have 2 mods that handles the player's death running independently. One needs a consistent framework to handle the alternatives.

Hm, I've used various "essential player" mods before, but I don't have any installed now. At the moment, I'm just keeping the bleedout threshold at 25% and it's working well enough for now, although regardless of the animation setting I use, I always do the kneeling pose in third person.

I downloaded this mod quite a while ago, before all the fancy updates. I then removed all the files related to the mod, but now, everytime I die, I will get knocked down, but I won't die nor wake up again. I'll just be sitting on my knees. I've tried uninstalling the whole game, but that didn't help.

Try making sure you're set to essential, and try setting the Bleedout Percent higher, so the bleedout can kick in before your health goes to 0 and kills you. That's how I solved it.

I get a CTD, cca 2 minutes after loading savegame. Well it's probably because of the load of mods I'm using. If somebody have any idea why is that, please reply. It really troubles me, because i even did a czech translation, before trying the mod on my own. First I thought that it's crashing because I accidentaly edited something what I shoudn't, but I tried clear, not translated version too and it still crashes. I was using No Death mod before, but I uninstaled according to instructions.

And what makes you think it has anything to do with DA? I see you have many script heavy mods, so I'd be more concerned that it is just too many and not any mod's fault in particular. As much as I love Skytest, this was one of the first mod in my list that had to go for performance reasons. That was a while ago though, I don't know if it has gotten better.

I think you mean that you've uninstalled and are still essential (in the savegame --- so it doesn't matter if you de-install and re-install Skyrim). Well... you probably didn't follow the uninstall procedure then. You can set the player un-essential in console with "setessential 7 0" or something like that if memory serves.

Don't take it the wrong way, I'm not blaming this mod for it, I'm just asking if someone happens to know why it could be happening, what other mod could be incompatibile with this one, because the game starts crashing when I turn on this mod and actually goes fine with all those other mods, so it has to be conflicting with some other mod I'm already using, I'm sure it wouldn't crash on clear save with no other mod, but I don't really want to start a new game and I can't disable all those mods because the save would probably s*** itself

I actually tried uninstaling Skytest, now the game doesn't crash after certain time of doing nothing, but right after the "you got your butt kicked, random scenario will now trigger"... I'm gonna try uninstaling another mods and maybe I can get it to work even in a messy game like I have
Edit 2: After lots of disabling, enabling, dying, clearing saves and crashing I found out its the Captured addon wich was probably incomatible with some of those many mods, this works fine with every other mod, at least I think.

I dont understand how all the random chances for on-blackout events work?
Currently im having a problem where the only on-blackout event that seems to be triggering is the friendly/hostile npc that appears, Its the only one thats triggering, in addition my player character is not being relocated away from the hostile npcs, so time progresses fast, friendly/hostile npc appears in the middle of the npcs that just knocked me down, and crazy stuff happens.
Do I have my on-blackout events set wrong?
Im currently experiecing this issue with the default skyrim event of a group of soldiers that killed imerial soldiers and are pretendign to be imperial solders demanding gold.
