I haven't had any problems with that quest other than Thaz getting stuck in a loop casting spells, I just ignored him, did what had to be done, then killed him.

I wonder if it's anything to do with your system, I've been reading that some people have crashes at various points in the game and despite the bugs which the team have fixed asap  I can honestly say I've never had any crashes.

Have you reported it on the BW projects page?

Not had any crashes. the only issues I had related to display of faces and heads during conversations - sometimes (not always dammit) the image floated half way off the screen or was blocked by foregroung scenery

When I first started DT I found I was getting some crash issues and what that tells me from my 10 months of playing this game is that my mods are overloading the system. I dumped a bunch of things, area mods that I seldom or rarely use and that helped tremendously. Of course I still have the usual Bioware created issue of everything working fine for a week or so and then crashing 5 or 6 times in the course of a day but that isn't a mod issue, that's just poor programming.

The problem I'm having is when the Lethe sends you into the fade to relive her memories, no matter which mage I side with in the lab the game shuts off right after the battle.

If you have a lrage number of mods running, I suggest you try deactivating as many as you can reinstate them one at a time to see if you can identify the culprit. A bit hit and miss I'm afraid but you may find a solution - at least it'll give you plenty of exercise fighting the mages
