I had audio really low also if I set at maximum in option menu.
So I realize to go in SkyrimPrefs.ini (in Documents/My Games/Skyrim) and have changed the following line:
fAudioMasterVolume=
For me, the previous value was 1.0000 so I raise it to 3.0000, but also 2.0000 work fine.
So, now I have
fAudioMasterVolume=3.0000

I hope can help anyone.
IMPORTANT: Don't make my same error. There are 4 zero after the point and not 3.
