Is it possible to add a new difficulty mode?

Title, basically. I know of mods that change incoming/outgoing damage numbers, but what I'm hoping to do is create a whole new option in the settings menu. Is that possible? I've never seen a mod that does such a thing, so I'm worried it might be hardcoded. Does anyone know anything about this?
