Damage resistance and locational damage

How does locational damage actually work? Head, limbs and torso use separate slots, but they just sum up to overall DR. But limbs can be crippled, like in previous Fallout games. Does this mean that, for example, good torso armor is a better protection from head wounds than a helmet (cause DR is higher)?

 

And what about extra slots? If someone makes a mod using some other slot, like a shield using "shield" slot (59), does it also count towards damage resistance?
