Weird Pip-Boy Glitch Issues

I am running a heavily modded Horizon game. And today I went to open my game and this happened:

I am not exactly sure what happened. It was working fine. I had my game rolled back and not updated, and even tested with the latest patch. Nothing so far that I have been able to figure out has fixed the situation. Here's my LO:
