In my imagination I see an NPC who can gift various things but at significant costs. Such as reviving a defeated boss (like Harkon or Miraak) as a follower, or granting you something you something you missed out on due to a choice you made. But in exchange he reduces some of your stats, kills your spouse, removes a home you bought, or otherwise causes trouble in your world. Or maybe you could bargain with him by trading filled black soul gems of varying amounts. I don't see this as a stand-alone mod but it could be a piece of a larger mod.
