Both sides of combat become hostile if I intervene

Having a fairly major issue with Skyrim. If I intervene in any combat, both sides immediately become hostile. For example, I attacked an Imperial Patrol escorting a Stormcloak prisoner, and the prisoner immediately became hostile and joined the Imperials in attacking me. I reloaded the save multiple times to make sure it wasn't from accidentally hitting him with my greatsword's wide swing, but same results every time. Thinking it was a civil war issue, I uninstalled the Populated Civil War mod. However, merely minutes later, when I encountered the companions attacking the giant outside Whiterun, I joined in as any warrior character does- but they also immediately became hostile. Again, loaded previous save to test this four or five times with the same results. I've had zero luck finding anyone with this specific issue by searching the web. Any suggestions or solutions?

EDIT 2: DEBUGGING
I have been debugging my modlist for about an hour now with no solution. I've tried removing every mod that could have anything to do with NPC behavior and so far, no solution.

It'd be helpful if you posted your load order in spoiler tags

 

I know I had a tough time using one magic cloak spell that created a soul trap aura around me, because it set off every hostile and non-hostile NPC because they took the spell as an attack.

I will post my load order tommorow morning (I woke up late and have to scramble to work). However, today I did try and experiment with this fully vanilla, all mods disabled. Still occurring. I've tried validating my steam files but that didnt work either. Since it doesnt seem to be any mod, I have no idea where to go from here

OK, now if you removed the mods, and loaded a save where you were using the mods, then the hostile 'bug' is no doubt, baked into the save.

Which no amount of adding or removing mods will fix.

 

Try a vanilla game with a brand new character and see if that happens.

Alright, so update after some testing.

Not sure what I did, but the problem is mostly fixed. Combat encounters seem to go fine now. I want to do further testing, but the two I was involved in that didnt involve the civil war (Companions vs Giant, whiterun guards vs bandits) went fine. However, there is still an issue with the civil war factions. I will have to test actual clashes between the two sides, but tests against killing a patrol to free the prisoner they're escorting still result in the prisoner going hostile. Will use my two test characters to experiment and see whats the issue

 

Edit 1: Update, battles between civil war factions work fine, issue is solely with prisoner being escorted encounters.
