I use Amazing Follower Tweaks (AFT). It includes a dialogue option to manage a follower's gear, so you can freely swipe their "default" armor or outfit and give them whatever armor or outfit or other items you want. 

If you use AFT, I think the vanilla follower AI is still intact, so if they prefer light armor, they will still wear equip light armor if they are supplied with both heavy and light armor in their inventory.

Yes, a follower framework is your best bet. 

 

ATF as mentioned above, or EFF or NFF or or or or..... just search follower framework, and about half a dozen will pop up.
