I'm having trouble in Fo4 where Preston isn't speaking to give me the first step quest, he talks to me about sanctuary and then the dialog stops when he's about to give the next quest
I've tried New Save, New Install, Validate Files, Commands, Mods off, Load Order is sorted with LOOT

You've pasted your load order upsidedown which is fairly confusing. @_@

 

Anyhoo, first and foremost - the basics. How many saves do you have in your save folder?

How many saves for different characters?

You want to only have about 10 saves for ONE character at a time in your saves folder.

The rest you should move to a separate backup save folder you create yourself, wherever you want. Or delete them.

 

That aside, i'm suspicious of your Start Me Up mod. Try disabling that one and see if it works.

The Extended Dialogue Interface mod is also extremely problematic and old. It needs a remake. You may be missing a patch for it, make sure you have everything related to that mod.

 

If none of that helps, I also point the finger at the UFO4Patch.

You have a lot of mods. The UFO4Patch becomes much more problematic than anything if you've got more than a dozen or so mods. It should not be installed as a baseline.

If you encounter a bug that you dislike in the game, you should install a fix-it mod for that specific mod on an individual basis (and flag it as esl).

I think i've been through like 4 threads exactly like yours this week, and in every single one of them removing UFO4Patch helped.

When preston says "Anyways, i'm glad you're here", it's normal for him to take a long pause.

What's actually happening during that pause is - something is happening over at Tenpines Bluff. Someone's getting kidnapped or ghouls are givin' them trouble or Corvega's being a pain.

The pause can be quite long. A solid minute or more sometimes. I'm not sure how long you're giving Preston - but try just waiting a minute or two.

 

If he never speaks up again, try going to Tenpines Bluff first, and kill the settlers before talking to Preston.

 

Aside from that - your load order is not how i'd personally do it.
