Do you have an AMD card? With a recent update they included AMD gaming evolved which contains the program raptr which is an ingame utility for screen recording/streaming. This program causes New Vegas to hang on the Bethesda screen when DLC/mods are installed. My successful FNV boot rate was about 1 in 5 if that before I learned this and is 100 percent successful now that I kill that process before I start the game.
