Hello. I am getting the exact same issue. One thing I noticed was for me, this starts to happen after I visit the Zent Hideout at Waukeen's Rest. Did you noticed this on your play through? Were you able to find a fix?

Works like a charm brawnsun! Thanks! For the mod part, I believe the breezehome mods are the culprits. I've tried a new game without the said mods and she appeared normally. I do not have the better skill mod though, so I wasn't able to test that.
