why does my head keep exploding?

no like i mean i'm not even in the dlc. sometimes i just randomly explode as i'm walking in the mojave -
for whatever reason (haven't even played dead money yet) my head will explode every great once in a while and every time it does i nearly piss myself because it happens at the most random of times and so far never in combat. anyone have any clue what causes it?
