Help updating mesh swapper to Next Gen .984

To clarify, I can get the plugin to compile to a DLL, but I can't figure out how to get it to load into Fallout 4, how it hooks into the exe. -
Looking for help upgrading a mesh path swapping plugin, CustomMeshesPathF4 to Next Gen .984. I've tried repeatedly to do this myself, but I just haven't been able to figure it out.
