You are naked or are only wearing clothing, you can get killed with one hit except an unarmed punch. This applies to all playable races of the game including NPCs, even kings, and all other quest-dependent actors. So is life. I know this is combat related, but it should be an example of how far we would need to think to define realism in a game. I think for any approach to developing a realism mod one should begin with general parameters such as basic survival needs, base health, amount of food, water, and sleep per day, inventory, carry weight, basic unarmed damage, then gradually explore peripheral facets of the game world that keep true to the base values defined. One would for example need to think about how much more damage a dagger does compared to basic unarmed damage, to a playable race, in most cases human race. Non-playable races such as animals, undead, trolls, giants, would need their values adjusted according to that basic armed damage, but that's still further in the periphery. I guess when the CK is released modders will be able to dive right into the code and the guys working on various realism projects can get a good look at how basic values are defined in vanilla Skyrim. That's when the work can really take off. I do like the ideas of being able to set up a tent, being able to make fire, using tools to cook food, having a notebook to write custom notes, only able to carry two one handed weapons, or one two handed weapon at a time. No more hoarding weapons in the backpack and one set of arrows. Also requiring a pouch to keep ingredients and jewelry, though one could argue whether jewels, books, and ingredients should have any carry weight at all for it is also just as important to keep in mind that a realism mod should never get annoying.
