I use the popular mod RH_IronSights. It kind of works with a gamepad, but there are many issues. This is because the script for RH_IronSights uses the command "IsControlPressed" which has very limited controller support for some reason. I've tested, and the mod works perfectly fine with mouse and keyboard.
Anyways, does anyone have the modding knowledge out there to make a mod for fallout 3 that makes IsControlPressed commands work better? A mod to make input from a controller recognized better? (I don't want to have to download Xpadder).
