This even happened to my ps3 save as well as my pc

the same problem happens to me when i leave "the sanctum" through "exit to sewers" manhole as well but i checked my only inadequate component is the video-card and the rest exceeds recommendation, so any hints would be very appreciated i tried "coc" console command, but when i leave interior it just crashes again. the only place that worked was "ICMarketDistrictStonewallShields" only the main room works, private quarters and basement just crash the game.
