Game freezes when the player gets killed

Came back to FNV after a few years away from the game, did a fresh install and set up my mod list. Mostly ones I've played with before (with no major performance issues) but with some changes to account for new developments (Project Nevada is out, the 255 plugin limit extender is in, etc). I started up a game and got a couple hours into it and got as far as Primm with occasional stutter but no serious issues.

 

I ran into one when I was killed for the first time; the game hung up on the death screen with the heartbeat sounds and the blood spray and remained stuck there. The audio continues to play indefinitely but the usual death horn sound cue that precedes a reload never kicks in. The application is frozen entirely and can't be shut down without forcing it through a computer restart.

 

My first thought was that the Custom Health and AP Visuals mod might have been screwing things up, but disabling that had no effect. I've tried disabling a few of the others, launching from fresh saves, lowering my graphics settings, and all the other obvious changes, but nothing has worked so far. The 4GB patch and the NVSE plugins all seem to verify as having been installed properly.

 

The freeze on the death screen replicates 100% of the time; I am experiencing no other random crashes, only this one very specific one under that specific circumstance. Does anyone have any idea what might be the problem here? Posting load order below.
