Please help! weird texture showing where limbs are shot/cut off

Hi,

As you can see this weird circular texture at where the wounds texture are supposed to be.

 

This showing up at the tip/stump of dismembered arms and legs.

I have loaded the game up with no mods; had a clean installation as well but this weird texture thing still showing up no matter what i do.

 

Can someone help please, I have already searched on google but seems no one else had this issue.
