Callighan
You might have not placed the needed meshes in the correct folder. That diamond "!" sign means the game can't find the clothing mesh.

Callighan
The mod should have a mesh folder in the zip file. Extract that folder into the game's data folder.
Smosh
The problem is that you haven't followed the instructions on how to get it to work.

Derkins
Actully I don't agree with what most of you say. I have had this same problem a few times and I looked and everthing was where it should be and I still had the same problem. One was with a gun the other was making Charon into a human female. I checked a few times and removed and placed the mod back in the folders a few times and I stil got the same problem.

One thing that was happening for me was that i had forgotten to go through the entire Nif a route all the textures inthe right place so when it was in game it was looking for a file that wasn't really there. Open all the nifs you have for the model (_n, _sk, etc) and make sure you go through EACH of the subtrees and find any and all referance to texture place. find the file and link to it with the flower then get rid of anything before the word texture (I.E. C:\programfiles\bethesda\fallout3\data\) or just type it out if you know it by heart. then try it, like i said it took me a couple of days of going over and over everything before i figured out there was more then 1 or 2 sections that pointed to the Texture file >.<
