FNV freezing and crashing frequently

I'm sorry, you guys must be tired by now of these topics, but I'm really at my wit's end and couldn't find anything about this.

My FNV keeps crashing and freezing constantly, it is not very heavily modded, but I do have more mods loaded. Here is my load order:

The thing is, it crashes at irregular, but quite short intervals, mostly freezing and crashing at loading screens.

Interestingly, it just started crashing this frequently after about 90 hours played, before that it was mostly stable.

What can I do ? I tried the 4GB loader, but it doesn't seem to help. I tried verifying the integrity of the game cache, and, although it downloaded some files that it said were corrupted, it didn't really help either.

My computer specs:

AMD Phenom X4 II 810 @ 2,6 Ghz

4 GB RAM

GeForce 9600 with 1 GB VRAM

Windows Vista Home 32bit
