Some guy in checkered armor shoots you in the head with a crossbow, took the chip, and you end up buried in the Falkreath cemetery. Fortunately, a nearby dwemer sphere saw the whole thing, dug you up, and revived you. Now you are on a quest to track down the guy who shot you and get that chip...
