Question regarding the feasibility of adding new weapons, items, materia, etc

Most mods I see are things like outfit or texture swaps, balance changes, and performance modifiers.

 

But what really gets me interested is mods that add new options and content to a game. For those of you who've done some modding, is it feasible to be able to add new items, weapons, and materia to the games?

 

As an example, while I understand it may be difficult to add full new elements, such as the Water and Quake spells from the franchise, I could see the Ruin spell line being introduced since it's non-elemental and wouldn't need an overhaul attached to it. It would be neat to see new weapon options being added for the characters as well, and perhaps consumable items from across the franchise.

 

But are mods like this really feasible at all?
