Combat Failures Mod - Quick Hack

The time has come to implement failures for quick hacking, to make it more unpredictable, as I did with weapon jams and Cyberware failures. I have a few ideas, but would really like some from the community.

How would you like to see quick hack fails?
What should the consequence be?
Should it have multiple tiers, like normal and critical failure?
