Graphics problem, loss of focus.

Hello.
I have problem with graphic in my game.
When I play my game loses focus and graphic is glitching to potato form and back to normal.
It happens even while talking to NPC (see link with video in google drive)

I think I saw mod that force close up graphic to be sharp while all distant was blurry.
But I cannot find it anymore.
Anyone is able to help a gamer in need?
