Needing help again, random CTDs (follows same pattern)

First and foremost, I notice this happening right before I CTD:

When the encircled "graphical anomaly" occurs my game doesn't crash instantly, I can close the inventory menu and play on for a few more minutes then it CTDs, however, if I immediately open my inventory or favorites tab again right after that "anomaly" appears, my game CTDs instantly.

 

Anyone have any ideas why that is happenning?

You are nearly over the mod limit
