Resetting quests - maybe pursuit of new game+

I get that quests can't be simply reset.

 

There are variables that are updated along the quest that might track things like whether the player picked up an item. So just changing the quest stage might screw up the game by putting it in unanticipated state - the quest fires and quest scripts expect a variable at a starting value ... but it's not. Is resetting a quest a question of having a variable map for each quest so that all the quest vars can be reset back to starting values? Are there hidden variables or other factors that complicate this further?
