Stop icons on custom hotbar from being removed when no longer available (weapon disarmed, prepared spell changes, etc.)

Massive quality of life for people who use custom hot bars is for actions to not disappear when no longer available - sick of having to re-add weapon and ability actions etc to my hot bar if I get disarmed. Just grey the action out or something
