Trying to Retexture items for a mod and they keep showing up as the rock ground texture

I'm not sure what I'm doing wrong, I'm creating a mod that involves retextured armor and a flag. (being used to create new items).

I'm new to the modding, i created a retexture a few last year but I'm basically starting from scratch.

if you can, could you please tell me how to fix this.
