Werewolf transformation while on horse (can't move)

I was force transformed while on a horse, and now I cannot look up or down and when I run it does not run toward the focal point, it runs sideways and eventually gets stuck. 
I tried to fast travel, change pov, reset, etc and now I am stuck in the floor at the lowest angle 1st person 

Only add mods are: FULL SKYRIM CONTROLLER (NOLVUS), & MiniMap 

Not the controller, as the keyboard behaves the same 

 

Please assist, stuck in the floor and not sure what to do (don't have a recent save to revert to)
