Overhaul modifications like The Old Realms & Realm Of Thrones tend to disable Birth & Death Options due to the developers not wanting it for varied reasons, most typically lore-based. These options are permanently disabled, leaving players and the greater world unable to die in battle, age or give birth and no mods exist to re-enable these features via overriding these overhauls.
