This is a shoot from the hip answer, I may be wrong, but I believe it is how much higher or lower in level the NPC or creature will be than the PC. For example a level 20 Player Character encounters an NPC with offset of -2. The NPC will be level 18. If he encounters a creature with an offset of 3, it will be a level 23 creature.

David nailed it right on the head. Level Offset is the number of levels that it WILL be above or below the player. Setting -5 Offset will mean if the player is 20, the NPC will be 15. If it's set to 5, then the same npc is 25. It's a great way to keep things difficult for the player, no matter their level.

I'm actually thinking of changing the levels and offset of the "main NPCs", like Mannkar Cameron, the arena opponents and so on.

 

I have both OOO and MMM.

 

What I'm wondering, is will need to change the NPC offset and min/max in both the OOO and MMM for OOO esp? Or just the OOO one? I assume it's both to make them not conflict but not sure.

If it's only select NPCs from Vanilla (not added by either of those mods) then you only need to make the changes in the one you have loaded LAST. If it's an NPC added by either of those two, then you need to make the changes in their respective esp.
