If you'd make everything exterior, you'd still not get loading screens, but ugridstoload would control how many cells are loaded, so you wouldn't have stuff all the way down to Riften in your memory. Technically that's not "all in one cell", but I'm guessing it's what the OP meant. Anyways, you're right about lags, the cities take a long time to load anyways, now imagine having every clutter item from every house loaded on top of that... *shudders*. Plus, there's the tiny problem that the interiors of buildings very rarely match the outside - they're larger inside. Not only would one have to rebuild them all from scratch with the CK, I also don't think the interior and exterior meshes match up well, so on top of it all the architecture models would have to be redone. Bottom line: crazy amount of work, lots of unresolvable issues and quite possible entirely impossible. That said, there's a mod called Open Cities or something like that, it makes the cities use the same worldspace as the rest of Skyrim, so part of what the OP wanted at least.
