Customizing my characters in the middle of a game

Would it be possible to make a mod that allowed the players to open up a character-creation-screen anytime they want to during gameplay? This way one would be able to customize ones own player character's appearance any time they want to. I know I want to sometimes, maybe simply changing their hairstyle or perhaps changing something I've grown unsatisfied with during playing.

 

But most importantly, and the actual reason that I'm requesting such a mod, would be the possibility to customize and change companion's appearances completely to my liking as I recruit them in the game. It would be nice to be able to change their looks sometimes on later playthroughs, instead of always having the same faces whenever i play the game again.

I know it's probably quite simple to change the companions' faces via the toolset before even starting a new game, but as for me, I'm one of those who to my frustration have never been able to use the toolset due to the problems with the toolset and compatibility-issues with the sql 2005 manager or whatever the name was, despite many, long and hard attempts at making it work. So without a toolset I don't get far... I don't know how many people there are, who like me, can still not use the toolset, but I'm sure that such a mod, if possible would be something many would find interesting.

 

I really hope it can be done, if anyone at all are even willing :)
