It only activates after you leave vault 112 or 108, and you have to be near megaton for it to start. There are some mods that make it so that you arn't from the vault, such as alternate start. In my experience that caused the issue of not triggering trouble on the homefront unless you went through 'Escape!'. This sound like what happened to you?
