How can I give an NPC a sex-change without getting this problem?

I'm trying to make all the dancers and such in Gomorrah into men. To start, I went in and unchecked "Female" on all the female prostitutes as well as Dazzle and Joana. But some of the male dancers now have white skin on their faces, but darker skin on their bodies. These ones I assume are female-to-male, considering both Joana and Dazzle suffer from the same problem (Dazzle is notable because I checked her face before saving, and in the GECK she had a hispanic male's tan skin with blonde hair, but she has pale skin with blonde hair in-game).

 

What causes this and what is the proper way to sex-change an NPC?

Off hand, I would be tempted to think that this is due to changing values related to an actor who already exists within the save you are using. Try testing it from a fresh game.
