A more complete answer... when developers place a spawn point, they can pick what types of creatures spawn there and at what player level they will start showing up. It's entirely up to the map designer to choose. In FO3, you would have spawn points that would start out as mole rats then progressively start spawning in harder and harder enemies (all the way up to Albino Radscorps if you had Broken Steel installed). Other spawn points were more limited in the range of enemies that they would spawn in (such as the Super Mutant spawns in the Mall or out at Hallowed Moors Cemetery). In FO:NV, the devs tend to only put a single creature type/level at a particular spawn point. Which gives the illusion that leveled enemies were removed. It's still possible to have an area scale to the player's level, but the way the spawn points were laid out and the internal lists created, they didn't take advantage of it or specifically chose not to (more likely the latter). (I've only peeked at a handful of the FO:NV spawn points. Oblivion was level-scaling insanity, Fallout 3 took a more moderate approach although most spawn points were kind of weird that they would spawn low level enemies then spawn higher level enemies as the player leveled up.
