I have a similar sound problem, and i found two solutions:

1) play in windowed mod ( i don't to this because the game screen don't fit well in my monitor )

2) after the game launcher go to windows task manager select the falloutNV.exe and set is it's priority as Hight ( right click on its name the last options is define priority) aand back to the game.

 

i alway do the second option and no sounds issues.

BOOSH!!! That did it! Kudos for the simplest of resolutions ever! I can finally play NV without gettin in a piss.
