Yes its very vague and there are several projects working towards new communities in-game.

 

And "City" in Fallout is about the as vague as you can get, Look at Novac its a city the lives in a hotel, and they don't even use all the rooms.

 

However I'm assuming you mean create a city on par with New Vegas. In which case what kind?

 

Underground? Like The Mod Station -13- This idea is often good since it allows the modder to put it easily in the game as all they have to add is a hatch or cave to the mojave.

 

Vault? Same as the underground, but playerswill demand a good backstory and "Oh its a control vault" will gain you instant hatred by story lovers.

 

Aboveground (In Vanilla World Space) - Lots of problems, as depending on location and size you have to check for conflicts with mods, move creatures, etc. Or you can make multiple cells and somewhat avoid some problems.

 

Aboveground (Custom Worldspace) - Worldspace creating is buggy as hell, thats putting it lightly, then good terrain and city layout is required, and also it has to be somewhat believable and convient to get to, so no magic teleporting glasses. Personally, and I'm sure others don't appriciate trying to take shortcuts when they damage immersion.

 

Sky city - Hasn't been done as far as I know, but obvious difficulties are sky problems, immersion, navmesh and npcs falling off and dying, plot, etc.

 

Famous Cities - Good story and must have unique sights and landmarks, creating a practical duplicate of New Vegas and calling it New Reno, will earn you instant Fallout hatred from Classic Fallout players.

 

Obviously new communities are a hassle, otherwise we'd have a lot more, they all require fun and interesting storylines, (Since he wants to visit a place in the game where no one talks, no story, just go in kill something, then never ever go back to the boringness)

Good design, interesting dialog and npcs, custom content is usually expected. etc. etc. etc.

Now After spelling all that out if you want to work hard, gather modders, and be willing to do a good amount of work youself, its possible, most things are if you work hard.
