SirTwist
When you use a bash, or merge, patch, you need to put it at the end of your load order.

When creating it, do I merge EVERY SINGLE thing it can or only a specific list?
SirTwist
You can do either. How many mods are you looking at running? 100+? If so, you might be better off making merged mods. Merge certain ones, like all your clothes and armor into one mod. All your weapons into another. And so on. There is some information here on the forums. There are videos, as well, on youtube. And there are also websites. You might be able to find more than I can. However, here is a good guide to have on hand while doing things, and may help even further: http://wiki.step-project.com/User:Essarrbee/FalloutNewVegas

SirTwist
I am glad to have helped. I have spent a lot of time looking at things, reading, and asking people questions. Have some fun, and play safe. And if you need something, please ask.
