Weapon realism - bleeding

In real life, you get shot with a bullet and you bleed. I like to aim for as much immersion/realism as possible when playing FO4, so I thought that, at minimum, all ballistic weapons should cause at least a little bleeding. You could argue that energy weapons might cauterize the wound to prevent bleeding.You wouldn't want to make it overpowered, of course. The game is easy enough as it is, even with handicapping myself with as many realism mods as I can find. But, I do believe it would be balanced/possibly more difficult given that enemy weapons would be causing you to bleed as well, and there will be more of them shooting at you than you at them. I have FO4edit, I poked around at possibly doing it myself, but am not sure it's even possible, and if it is I couldn't figure out how to make it work. Ideally, the bleed effect would be based on bullet size and/or total damage. I wouldn't mind doing to work to make the mod if someone could let me know how/if it is possible. Thanks.
