What is that?

Also whenever I open FOMM it ask if I want to make the Fallout.ini not just readable. What is Fallout.ini and do I want that?

I'm kinda new to modding and want to understand but this is confusing for me

Thanks and much appreciated.

Geckcustom.ini contains settings for NVGECK which is the main tool for modding.

Fallout.ini contains settings for running the game and related programs ... by default it is read only but for some modding procedures you need to change some settings in there, so you need to make them "not just readable". There are two copies of Fallout.ini one is in your Documents/My Games/ folder and the other is in your Fallout New Vegas folder under Steamapps/common. Don't ask me why they needed two copies of the same file in totally different places ... I don't know ... it's probably the same reason I need to click 4 or 5 times to confirm my age everytime I visit the official New Vegas website. I think its called design by committee or something.

Speaking from the point of view of one who knows very little about these files, I edit them as required when I'm following instructions or following a tutorial on a particular modding technique and they tell me to make sure a particular setting is set to a particular value ... otherwise I leave them be.

If you have to make changes it is always recommended to backup your editied versions since the game loves to overwrite the ini files and reset everything to defaults without warning :D

In this case backup = make a copy and give it a new name - something like fallout-ini.original. Then if you, or some mod totally borks the ini you can rename the copy and be back in operation in minutes.
