Breezehome CTD's usually happen because the Hearthfire dlc deletes the navmesh or so I've read throughout the internet.

This mod : http://www.nexusmods.com/skyrim/mods/27053/? will help fix it but OP has CTD when exiting Dragonreach.

Not sure what causes that but I did notice that there are no official patches in your load order.

Do you have them or did you just not bother to include them in your load order?

If you don't, I urge you to get them as they fix an awful lot of bugs.

I see no Update.esm in your load order either.

Btw, CTD's when leaving/exiting a cell can also be caused when you are out of vram due to too many textures.

Yeah I noticed the missing patches, too. I figured after playing 120 hours without them, op must be very lucky. And must have gone through Whiterun before doing the low level quests, then something changed that started ctd's. So i suspect Breezehome.

Regardless, the patches are a good idea and may fix this.

Edit: assuming it's not the Breezehome replacer mod (how can you be sure?). Have you never been to Whiterun before level 60? I'd assume it's something in Whiterun exterior that changed after 120 hours of play. To be able to help, we need a clear picture.

WITH Breezehome fully upgreadeable on it already. Sorry for late replies but I dont have the time work takes most of it and I doubt its VRAM

System Specs:
i5 4690k

8gb Crucial Ballistic

Z97 MSI PC MATE Mobo

Gigabyte Windforce R9 290

EVGA SuperNova 750w

Coolermaster 212 Hyper Evo

No overclocking done so I am confused whats happening cuz this PC is running cold due to the amount of fans I have so that R9 290 even under load doesnt cross 68 Degrees C

I recommend you turn of autosaves until you solve ctd.

SkyrimPrefs.ini in your Mod Organizer profile or in Documents\My Games\Skyrim

[MAIN]

bSaveOnPause=0
bSaveOnTravel=0
bSaveOnWait=0
bSaveOnRest=0

Your graphics card has 4gb vram so I recommend these settings in your enblocal.ini

[MEMORY]
; ReservedMemorySizeMb needs to be as high as you can tweak it an still be stable. With 4gb gpu I set it to the max, 1024
; Default is 64, max is 1024
; Start with 512, increase by 128 to tweak higher
ReservedMemorySizeMb=512
; VideoMemorySizeMb can be increased to 8192 to use some of your ram for graphics, but probably will cause stuttering (better than ctd)
; VideoMemorySizeMb=3968 <- best for no stuttering
VideoMemorySizeMb=8192
; Compression adds some gpu overhead but saves vram. With 4gb gpu I recommend turn it off
EnableCompression=false
[LIMITER]
; WaitBusyRenderer may help stop ctd. Makes it worse for some people. See this guide
WaitBusyRenderer=true
