All NPCs completely naked

Just started playing again and I started getting this error today, where basically for no apparent reason all NPCs show up as completely naked. Even if I disable/deactivate every single mod I have in FOMM this still occurs. It only seems to go away if I reload an earlier save, although eventually it happens again after a loading screen even if I'm running with no mods and boot up vanilla New Vegas through steam.

 

I'm reinstalling now but I'm sort of wondering what would cause this? I do enjoy playing with mods and sort of want to nail down what causes this so it doesn't happen again.

So apparently reinstalling didn't fix it and no matter what save I use or game I start eventually after a loading screen NPCs stop wearing their equipment...

Do not use Fook and Project Nevada at the same time
