basically the idea is for the player to be contacted by the institute in the fo3 game side of things. He or she is to travel with kelogg to vault 111 and retrieve something. When things go sideways kelog shoots the parent and escapes with the kid. You are left to save the now frozen hero of fallout 4 as well as thaw out and resettle some of the citizens of vault 111. only to have about half die in this scenario.

Probably better to do it via a FO4 mod as everything I'v read in regard to TTW and nexus indicates it's problematic at best and against copyright policies at worst.

Plus the commonwealth assets would be available in FO4, and the FO4 geck is supposed to be out this month. But if you stay strictly in the vault and never have a need for commonwealth assets you could probably get by with FO3 assets. But you don't really want to "save" the hero, as he was preserved as a "failsafe" (pure genome or something) in case things went wrong with Shaun, and of course the timeframe requires those intervening years where Shaun matures and takes his place as director of the institute (vital to the story line, that the lone survivor stays "on ice" during that period). So I think your sort of left with a minor conundrum. You need to fabricate a reason that the other vault dwellers are rescued and relocated but the lone survivor is left in cryo. And of course then, s/he would no longer be the lone survivor.

 

Anyway just a few points to consider, not trying to thread cr*p.

Oh, and just a quick addenum. Another reason to make a FO3 only mod version, as opposed to a TTW version (which is problematic enough in and of itself) is that you probably wouldn't need any FONV assets. it's much more likely resettling the vault dwellers would be done in the DC area (400ish miles from boston) rather than resettling in the Mojave (2700 miles). That's assuming you don't just wait for the FO4 geck and use the commonwealth as the settlers new home.
