display/resolution issues? (not really sure what the issue is)

im not even sure what to call this issue im having all the sudden- anything that pops me out of the game messed with the resolution and display. For example, the occasional warning i get during loading that i may want to change the display settings will cause it, alt tabbing out will cause it, alt tabbing out while in the save menu REALLY messes it up.... when it kicks in, the graphics suddenly look really crappy and if in a menu of any sort suddenly there is an invisible box that you cannot move your mouse out of no matter what you do.
