Login to the Fallout Nexus Mods page (not the forums). Click on your account name. Select "User Area" in the droplist. That will take you to a grid of 3x2 windows. The 4th window (lower left) is "Files". Select "Add a new file". Fill in the wizard template.

Note on the right it displays the remaining steps you need to take. The wizard template is step 2. The filename you give there is the name that your mod file will have on the Nexus. It does not need to match what you have on your disk drive.

You can get to the wizard template and review what you are going to need to fill out without actually committing anything. Take your time and gather everything together before proceeding.
